Home and Community Based Services


AFH offers in-home supports to individuals with disabilities living on their own or with their families. Our trained providers offer individualized care and support in skill attainment to increase independent living skills.

The Home and Community Based Services program offers respite, attendant care and habilitation services to provide support to individuals and their families in their own home. These services are individualized and tailored to strengthen independence and self-sufficiency. AFH direct support professionals (DSPs) will meet personal needs, engage in meaningful social activities and interactions, provide instruction on individualized goals and offer a respite to primary caregivers. Services are often provided in the family home. Families and members have choice in their DSP providers. The AFH team and families work together to ensure that goals and activities provide meaningful interactions and support maximum independence.

In addition, AFH offers congregate activities to incorporate social interaction and social skills training into the program. Activities vary by location.
